Waiver & Consent Form  

1. I confirm that I own the pet(s) or I was given authority by the owner for taking ownership of the pet to be taken care of by Safe At Home Pet Care .
    

2. I assure Safe At Home Pet Care that this pet(s) is currently in good health condition or stated otherwise. Pets with more medical issues or conditions may be liable for an increased rate due to the amount of extensive care needed. 

3. I confirm that my pet(s) has a complete and updated vaccination. 

4. I acknowledge that the pet(s) should have a safety collar. 

5. I confirm that my pet(s) doesn't have fleas or any communicable disease. 

6. I release Safe At Home Pet Care, its employees, and owners from any liabilities but not limited to injury, sickness, damage, accident, or death while in the vicinity under their care and supervision. 

7. If I am not able to pick up my pet(s) on time, Safe At Home Pet Care will be calling the owner and get authorization for the pet(s) to stay with Safe At Home Pet Care until the owner is able to pick up said pet(s) at the providers emergency pick up rate which will de discussed at said time. 

8. If my pet(s) needs medical attention, I authorize Safe At Home  Pet Care to have my pet(s) taken to their primary veterinarian. If the pets primary veterinarian is unavailable or the pet(s) need emergency care I give Safe At Home Pet Care authorization to take the pet(s) to be seen by a prelesional vet of my choice at my expense with my approval. I understand that I must have a payment method on file with said vet or their emergency care provider. If Safe At Home Pet Care needs to pay out of pocket I understand that i must pay back 110% of the bill due to the inconvenience fee that will be added of 10%. 

9. If you will choose to use our service during the holidays, please make sure to reserve those dates as soon as possible. Dates can not be guaranteed for holidays due to people booking up to 1 year in advance  

10. Cancelations for appointments for overnight boarding or extended pet care of at least 14 days prior will be subject to 25% cancellation fee which will be 25% of the total time you booked. Any cancelation of 10 days or less will be 50% of the total time you booked.  

11.  I confirmed that all information I entered in this form is true and accurate.
